How old was Utena when her parents have died - and which episode is it where you see her in a flashback, laying in a coffin of roses inside a dark church?

I believe it was seven years before the start of the series, with Utena being 14 by the start. So she was probably about 7 years old by the time of the accident.
As for the episode, I don't remember.

We've got a similar thread already, but to answer your question: The flashback was in episode 9, and I believe Utena's age at the time was never specified. The manga and light novels say she was six, but whether or not you want to take that as true for the series as well is up to you.
